An Advance Health Care Directive allows you to name an agent(s) to carry out your wishes if you should find yourself unable to make health care decisions.  Anyone over the age of 18 years of age should have an Advance Health Care Directive.  We can also prepare for you a health directive for your minor child(ren), so that if a parent is unavailable, an trusted individual is named to make health care decisions on behalf of your minor child(ren).

It's Important to think ahead

Advance care planning is not just about old age. At any age, a medical crisis could leave you too ill to make your own healthcare decisions. Even if you are not sick now, planning for health care in the future is an important step toward making sure you get the medical care you would want, if you are unable to speak for yourself and doctors and family members are making the decisions for you.

Advance care planning involves learning about the types of decisions that might need to be made, considering those decisions ahead of time, and then letting others know—both your family and your healthcare providers—about your preferences.

Get a head start

Anyone over the age of 18 should have an Advance Health Care Directive. Consider this situation- you have an 18 year old college freshman who is in a medical crisis and is unable to speak for him or herself.  Under the law, without written authorization, you are considered a "stranger" and would not be able to make any decisions on behalf of your child.  With an  Advance Health Care Directive, a person over the age of 18, names an agent and is able to have a trusted person make the necessary decisions.
